Parent Technology Media Information Consent Form-
 
The Parent Technology Media Information Consent Form (PTMICF) must be completed annually for all students. The PTMICF provides parents/guardians the opportunity to: - acknowledge acceptable and responsible uses of student technology, - give consent for a high school student to use social media, - give consent for the district to use your student's photo and/or video and, - give consent for the release of student directory information. Below are the link to the forms. You can fill them out online, print, and return back to school.
